To register a student for an independent study, complete this form after logging into your Adelphi account.

At the end of the form, there are instructions for how to forward the summary to obtain approvals and ensure proper processing. Please follow the instructions. After submitting the form, you will receive an email from Google Forms (check your spam folder if you do not see it in your inbox).  Please make sure to have the correct student email address in the form and the correct course number.
